Of the 3,000 individuals we care for each year, more than two-thirds live at home. That wouldn't be possible without the McLean home care staff, a caring group of skilled professionals who deliver quality services to clients throughout the Farmington Valley and West Hartford.
McLean's Home Care, in conjunction with our patients' physicians, develop multidisciplinary care plans tailored to both initial and ongoing needs. Their skilled nursing services involve assessment, evaluation, care planning, intervention and follow up. Their ability to coordinate the many facets of home care ensures a smooth delivery of services around the clock.
Home Care Therapy is provided through McLean's highly regarded rehabilitation department. We provide therapy for patients in their homes to enable safety and recovery from hospitalization, short-term inpatient stay or a change in their condition. All therapists are McLean employees.

Complimenting the work of our skilled nurses are certified Home Health Aides who assist with activities of daily living: bathing, eating, dressing, grooming and ambulation.
